Privacy (AVG)
To optimally support the therapy it is necessary for Mindful Rebalancing to keep a file, exclusively on paper. This is also a legal requirement. The notes include a brief synopsis about your health in general, information relevant to your treatment, a brief outline of any treatment, agreements and any changes to treatment or agreements. I adhere to the laws concerning confidentiality and I am the sole person who has access to the files. These are kept securely under lock and key during the work, and the ensuing 15 year legal requirement, after which they are carefully destroyed.
Mindful Rebalancing upholds high priority and standards regarding your privacy by:
- Guaranteeing that no-one, without your explicit permission, has any access to your information.
- Exercising the utmost care and confidentiality regarding any personal and medical information.
- Exclusively storing information on paper which is kept in a secure, locked filing cupboard.
When may it be necessary to share some information with a third party:
- To inform other health professionals, e.g. when therapy is concluded or when a referral to another therapist is required.
- For observation during my absence.
- During supervision, and in this case verbally, and with anonymization:
In all cases you will be informed first and your explicit (signed informed consent) will be asked. Should there be any other reason I may need to share any information your explicit informed consent will also be requested.
